How we can support your PCN
A Health and Wellbeing Coach works collaboratively with patients to set personalised goals and adopt healthier behaviours. Here’s what a typical engagement looks like:
Helps patients set realistic health goals — better sleep, weight management, stress reduction
Supports behaviour change — improving diet, increasing exercise, reducing harmful habits
Uses motivational interviewing and evidence-based habit-building strategies
Keeps patients accountable and tracks progress over time
Encourages a balanced approach — mental, physical and emotional wellbeing
Who do we refer?
This role is most effective for patients who are driving GP demand without a clear clinical need — including:
- Patients managing long-term conditions who need lifestyle support alongside clinical care
- Those at risk of developing conditions such as type 2 diabetes or cardiovascular disease
- Frequent attenders with no clear clinical cause for repeat appointments
- Patients going through significant life changes — redundancy, burnout or bereavement
- Those with physical and/or mental health conditions needing structured wellbeing support
